By Addressing Social Determinants of Health

 

PLACE MATTERS is a signature project of the Health Policy Institute of the Joint Center Health Policy Institute in collaboration with the National Association of Counties.  The PLACE MATTERS Initiative is conceived as the centerpiece of the HPI's work.  PLACE MATTERS will help communities create a set of indicators designed to measure progress toward the goals established by communities regarding whether and how health disparities can be and are being reduced.
