Chronic Ill Net. ChronicIllnet is the first multimedia information source on the Internet dedicated to chronic illnesses including AIDS, cancer, Persian Gulf War Syndrome, autoimmune diseases, Chronic Fatigue Syndrome, heart disease and neurological diseases. The site seeks to provide a forum for discussion where discussion does not usually exist; between different groups of people: patients and researchers, physicians and laypeople, for example.

Improving Chronic Illness Care (ICIC). ICIC is a national program of The Robert Wood Johnson Foundation committed to reforming the United States healthcare system by converting health care organizations to better chronic illness care. ICIC has created the Chronic Care Model, which summarizes the basic elements for improving care in health systems at the community, organization, practice and patient levels.

National Center for Chronic Disease Prevention and Health Promotion at the Centers for Disease Control. Information about chronic diseases and conditions, lists of publications from the National Institutes of Health and links to other sources of health information, health and education agencies, major voluntary associations, the private sector and other federal agencies:

Partnership for Solutions. A chronic conditions oriented Web site developed by Johns Hopkins University and The Robert Wood Johnson Foundation to communicate research findings to policymakers, business leaders, health professionals, advocates and others. The site contains chart books, research syntheses, and other aids that can be useful in describing the issues faced by the chronically ill.
