Thank you for visiting our PLACE MATTERS Initiative.  The PLACE MATTERS Initiative is conceived as the centerpiece of HPI's work.  PLACE MATTERS is a national initiative, designed to improve the health outcomes of  participating communities through a shared learning experience.  PLACE MATTERS is designed to develop community-based strategies to reduce health disparities and measure progress toward the goals established by communities that consist of 22 counties and 2 cities.
